Presenting NEXT ACO of Nature Coast

NEXT ACO of Nature Coast is a primary care centric Accountable Care Organization (ACO) founded by Jay Chowdappa MD with the intent to participate with the Centers for Medicare and Medicaid Services (CMS) Medicare Shared Savings Program (MSSP) starting on January 1st 2016. This MSSP ACO will share Medicare savings with providers who achieve the “Three-part aim” of improving the experience of care, improving the health and reducing Medicare expenditures.

Our goal is to provide much improved patient care outcomes by better care co-ordination for our Medicare Fee-For-Services beneficiaries throughout Tampa Bay area and adjacent counties. We are now inviting like-minded Primary Care Physicians to become Participants. There are no initiation fees to participate and the participant physicians have an equal opportunity to become Stakeholders in NEXT ACO of Nature Coast.
